As of April 1, 2022, Costa Rica does not require any type of COVID-19 specific requirement to enter the country anymore. No negative test, no quarantine, no travel insurance nor health pass.
All commercial establishments, activities, and events can operate at 100% capacity.
Although the use of a mask is not mandatory, it is recommended to maintain the protocol of hand washing and temperature taking, as well as compliance with health protocols in each tourist activity.
Costa Rica DOES NOT require a negative COVID-19 test or quarantine. **The insurance requirement has been removed on April 1, 2022 (unvaccinated tourists will not need to purchase the insurance anymore) and the health pass is no longer be mandatory since April 1.

Returning to the US
Effective December 06, 2021
If you plan to travel internationally, you will need to get a COVID-19 viral test (regardless of vaccination status or citizenship) no more than 1 day before you travel by air into the United States. You must show your negative result to the airline before you board your flight.
If you recently recovered from COVID-19, you may instead travel with documentation of recovery from COVID-19 (i.e., your positive COVID-19 viral test result on a sample taken no more than 90 days before the flight’s departure from a foreign country and a letter from a licensed healthcare provider or a public health official stating that you were cleared to travel).

Getting Your Test: Make an Appointment
The best way to get a COVID test in Dominical, Costa Rica is to call or contact them via Whatsapp or Facebook messenger and make an appointment. We recommend to make your appointment earlier than later as there can be long waiting times.

Costs vary for PCR Testing, Results in 48 hours. (Note that Antigen testing is no longer available at Medico Vargas.)
The nearest location for in house, Antigen testing is in Quepos about an hour north. The Hospital Metropolitano sends PCR samples to their own central lab in San José $85, PCR results in 48 hours, 9a-4p M-F and 9a-2p Saturday, phone +506 2519-9733, and does in house antigen testing ($55, results in 2-8 hours).
Required Form to Test
You will need to fill out an online form to get a registration number in order to make an appointment. It is highly recommended to fill this out on a computer rather than your phone. Navigating on the phone can be challenging.

Presenting Your Test Results
Upon checking into your flight, the airline will verify your negative result. You can show a paper or electronic copy, an email is fine. You will be denied boarding if you do not present a negative test. The name, date of birth, etc. on the test result must match your travel documents (e.g., passport).
The result must indicate that the sample was taken within three calendar days before your flight from Costa Rica. (See the CDC order for the specific requirements for connecting flights). It also must state: “NEGATIVE,” “SARS-CoV-2 RNA NOT DETECTED,” “SARS-CoV-2 ANTIGEN NOT DETECTED,” or “COVID-19 NOT DETECTED.”
